Premier Telehealth Subscription Service 

If you have a high deductible insurance plan, have an out-of-network insurance plan, or do not have any insurance but need to be seen fairly often (more than 4+ times per year), then the subscription service may benefit you financially. As your needs change, you can change your package selection, pause or cancel your subscription (some restrictions may apply).

If you have Medicaid, unfortunately, you are not able to utilize this service due to state regulations (yeah, we think that's dumb too). If you have commercial insurance and are likely to meet your deductible, or don't have a deductible to meet, the subscription based service is not likely to benefit you. 

No. The subscription based service is considered a self-pay service and if you had insurance we were in-network with, you would have to opt-out of us filing your insurance in order to do the subscription. We would be able to give you a superbill you could use to attempt to get reimbursed by your insurance. 

For brand new patients, you would pay a one-time $250 fee for your first month, which includes your online 60-minute initial psychiatric evaluation. Then you may choose either package #1 ($150 per month, 2 30-min sessions/month, 10% off one additional service) or package #2 ($100 per month, 1 30-min session/month, 5% off one additional service). 

Nope! This is a month-to-month subscription. To cancel, you must submit a request to cancel in writing and complete the appropriate form. No refunds are provided and fees will not be prorated. Your subscription would end on the last day of the month your cancellation request was received.

The monthly subscription cost only includes the cost of your telehealth visit with our provider Shannon. Your subscription cost does not cover the cost of your medications, labs, or any other aspects of your care. You do get a discount off of one additional service we provide per month!

Your visits and discounts do NOT accrue. Meaning, if you do not use it you will lose it. However, in many cases, using the subscription service will be cheaper than self-pay rates if you need to be seen fairly often. So, maintaining your subscription guarantees you are getting a significant discounted rate for services. 

You may pause your subscription for one month, once per calendar year. You may cancel your subscription any time by submitting a request to cancel in writing to [email protected] and completing our quick and easy cancellation form. If you decide to re-enroll, there will be a one-time $50 re-enrollment fee in addition to your monthly subscription cost.
